中国-澳大利亚自贸协定原产地证书
中国-澳大利亚自贸协定原产地证书，证书英文名称:《CERTIFICATE OF ORIGIN FORM FOR CHINA-AUSTRALIA FREE TRADE AGREEMENT》于2015年12月20日起施行。自2015年12月20日起，依照中澳自由贸易协定和国/家法律有关规定，申请人可以向各地出入境检验检疫机构CIQ、中国国际贸易促进委员CCPIT会及其各地方分会申请签发中澳自由贸易协定原产地证书。随附上述证书的出口货物依照中澳自由贸易协定规定在澳大利亚享受优惠关税待遇。
中国-澳大利亚自贸协定原产地证书基本概况及填写方式
(一)签证国/家
澳大利亚
(二)证书名称
证书英文名称:《CERTIFICATE OF ORIGIN FORM FOR CHINA-AUSTRALIA FREE TRADE AGREEMENT》，简称中国—澳大利亚原产地证书。
(三)签证产品
《中国—澳大利亚原产地证明书》的签发，限于已公布的《货物贸易协定》项下给予关税优惠的产品，这些产品必须符合《中国—澳大利亚自由贸易区原产地规则》。
(四)证书文字
证书内容以英文填写。
二、申请书
申请书的项目应如实填写，与证书相同的栏目应与证书内容一致。
三、证书编号
证书编号填写在证书的右上角。
中国—澳大利亚证书编号须填写证书种类识别字母“A”，例如:A15470ZC20390038。
四、中澳产地证填制
中澳产地证DI一栏:出口商名称、地址、国别Exporter’s name, address and country
此栏必须填明中国境内出口商的名称、地址、国名。该出口商名称必须经我局备案。
State the full legal name and address of the exporter in Australia or China.
中澳产地证第二栏:生产商的名称、地址和国别。Producer’s name and address
列明货物生产商的全称、地址、国/家。如果证书货物由多个生产商生产，应填写所有生产商的全称、地址、国/家，可增加附页;如果出口商或生产商希望该信息保密时，也可在该栏注明“应主管部门或授权机构要求可提供”(AVAILABLE TO THE COMPETENT AUTHORITY OR AUTHORISED BODY UPON REQUEST)”。如生产商与出口商相同，应填写“同上(SAME)”。
State the full legal name and address (including country) of the producer, if known. If more than one producer’s good is included in the certificate, list the additional producers, including names and addresses (including country). If the exporter or the producer wish the information to be confidential, it is acceptable to state “Available to the competent authority or authorised body upon request”. If the producer and the exporter are the same, please complete the box with “SAME”. If the producer is unknown, it is acceptable to state "UNKNOWN".
中澳产地证第三栏:收货人的名称、地址和国别importer’s name, address and country
该栏填写收货人名称、地址、国名,即注明澳大利亚进口商(如已知)详细的依法登记的名称和地址。
State the full legal name and address of the importer in Australia or China, if known.
中澳产地证第四栏:运输方式及路线Means of transport and route
填写运输方式及路线(如已知)，详细说明离港日期、运输工具编号以及装货和卸货口岸。
如出货后申请证书，应填写具体运输工具号;如出货当天或出货前申请证书，可只填写BY SEA或BY AIR等。
装运口岸应为中国大陆境内的港口，经香港转运可写SHENZHEN VIA HONGKONG。
卸货口岸应为澳大利亚港口。
Complete the means of transport and route and specify the departure date, transport vehicle number, and port of loading and discharge, if known.
中澳产地证第五栏:备注Remarks
可以填写客户订单号码，信用证号码等其他信息。如果发票是由非缔约方经营者开具的，则应在本栏注明开具发票的经营者的名称、地址和国/家。
The Customer’s Order Number, Letter of Credit Number, among others, may be included. If the invoice is issued by a non-Party operator, information such as the name, address and country of the operator issuing the invoice shall be indicated herein.
中澳产地证第六栏:商品顺序号Item number
按顺序填写货号编号，ZUI多不得超过20项。如多于20项应另申请一份证书。
State the item number; item number shall not exceed 20.
中澳产地证第七栏:唛头及包装号Marks andnumbers onpackages
如有唛头及编号，则注明包装上的唛头及编号。如无唛头，应填N/M字样。此栏不得出现“香港、台湾或其他国/家和地区制造”、或“见提单”、“见发票”等字样。
唛头是图案形式的可采取贴唛。需要贴唛的证书，应在申请书备注栏注明“M”或“贴唛”字样，不能留空。
State the shipping marks and numbers on packages, when such marks and numbers exist.
中澳产地证第八栏:商品名称，包装数量及种类Number and kind ofpackages; description of goods
此栏详列每种货物的货品名称，以便于海关关员查验时加以识别。货品名称应与发票上的描述及货物的协调制度描述相符。如果是散装货，应注明“散装”(IN BULK)。
应标明货物包装种类及包装总数量，总数量应同时用英文大写数字和阿拉伯数字表示。
当商品描述结束时，加上“***”或“\”。ZUI后应加上截止线，以防止添加伪造内容。
The number and kind of packages shall be specified. Provide a full description of each good. The description should be sufficiently detailed to enable the products to be identified by the Customs Officers examining them and relate it to the invoice description and to the HS description of the good. If the goods are not packed, state “in bulk”. When the description of the goods is finished, add “***” (three stars) or “ \ ” (finishing slash).
中澳产地证第九栏:对应中澳产地证第八栏的每种货物，填写协调制度税则归类编码(6位H.S.编码)。HS code(6-digitcode)
For each good described in Box 8, identify the HS tariff classification (a six-digit code).
中澳产地证第十栏:注明申报货物享受优惠待遇所依据的原产地标准。Origincriterion
(一)如该货物在一缔约方完全获得，填“WO”。
(二)如该货物完全在缔约一方或双方领土内由已获得原产资格的材料生产，填“WP”。
(三)如该货物在缔约一方或双方领土内使用符合产品特定原产地规则及其他有关要求的非原产材料生产，填“PSR”。
For each good described in Box 8, state which criterion is applicable, according to the following instructions. The rules of origin are contained in Chapter 3 (Rules of Origin and Implementation Procedures) and Annex II (Product Specific Rules of Origin) of the China-Australia Free Trade Agreement.
中澳产地证第十一栏:毛重或其他数量Gross or netweight or otherquantity
货物毛重应以公斤计算，也可按照习惯填写其他能表明货物具体数量的计量方式，如体积、数量等。
中澳产地证第六栏的序列号、中澳产地证第八栏的品名、中澳产地证第九栏的H.S.编码、中澳产地证第十栏的原产地标准及中澳产地证第十一栏的计量单位要一一对应。
State gross or net weight in kilograms or other units of measurement for each good described in Box 8. Other units of measurement (e.g. volume or number of items) which would indicate exact quantities may be used where customary.
中澳产地证第十二栏:发票号及日期Invoicenumber anddate
此栏不得留空。为避免误解，月份一律用英文缩写，年度要打四位数。此栏所填发票号日期必须与发票一致。发票日期不能迟于中澳产地证第十三、十四栏申请签发日期和中澳产地证第四栏离港日期。

中澳产地证第十三栏:出口商申明Declaration by the exporter or producer
申请人须在此栏加盖经检验检疫机构备案的出口商/生产商中英文印章及申报员手签，并填写申报地点和申报日期。此栏的申报日期必须是企业实际申请日期，与中澳产地证第十四栏日期相同，且不得早于中澳产地证第十二栏日期。
证书进口国已经打印为澳大利亚。
The box must be completed by the exporter or producer. Insert the place, date and the signature of a person authorised by the exporter or producer.
中澳产地证第十四栏:签证机构证明Certification
此栏由签证人员审核证书无误后，签名并加盖公章。签证人签名必须清楚、并留有足够的盖章位置，印章与签名不可重叠，如无特殊要求，只签一份正本，不签副本。
此栏的签证日期必须是企业实际申请日期，与中澳产地证第十三栏日期相同，且不得早于中澳产地证第十二栏日期。
The box must be completed, signed, dated and stamped by the authorised person of the authorised body. The telephone number, fax and address of the authorised body should be given.
中澳产地证其他注意事项:
后发证书的签发
货物未能在货物出口前或出口时签发的，可凭货物报关单及提单复印件(或电子资料)申请办理后发证书，证书补发有效期为货物装运之日起一年内。证书需在“供官方使用栏”标记“ISSUED RETROSPECTIVELY”。
更改证书的签发
证书签发后出口商发现证书错误可对证书进行更改申请。除申请更改的栏目外，更改证书其他各栏目与原证书保持一致，“申请日期”及“签发日期”也应与原证书一致。并应提交原证书正本。
重发证书的签发
如原产地证书被盗、遗失或损毁，在出口商或制造商确信此前签发的原产地证书正本未被使用的情况下，可签发经核准的原产地证书副本，并在“供官方使用栏”注明“CERTIFIED TRUE COPY of the original Certificate of Origin number ___ dated ___”。签证时需提供原证书正本复印件及相关的提单、报关单正本复印件。
重发证书其他各栏目与原证书保持一致。“申请日期”及“签发日期”应为企业实际申请日期.
证书有效期
自证书签发之日起一年内有效。
